Our programs are designed primarily to serve people with chronic mental health challenges.
The Collaborative Solutions admissions process begins with referrals. All referrals to the Collaborative Solutions programs are coordinated with the Department of Mental Health. The Department of Mental Health evaluates potential referrals using a prior authorization process; the approved prior authorizations are sent to Collaborative Solutions as referrals. We do not accept referrals outside of the ones prioritized for Collaborative Solutions by the Department of Mental Health.
We accept referrals for individuals who are currently involuntarily hospitalized in a psychiatric facility within Vermont. Referrals are evaluated based on a number of factors, including but not limited to: behavioral safety, medication adherence, medical stability, and the recommendations and clinical assessment of the referred individual’s treatment team. We do not accept individuals under 18 or individuals who meet a nursing home or skilled nursing level of care.
The admissions team will review the information sent to them from the Department of Mental Health. If our program is an appropriate placement for the potential resident, the team will schedule an online or in-person interview. Our goal is to make sure our program is suitable for the applicant, to ensure we meet the needs of each resident in the program and to create a safe environment for everyone that we serve.
