Our programs are primarily designed to serve people with chronic mental health challenges who are hospitalized but no longer need hospital level of care. Occasionally, we also take community referrals.
The Collaborative Solutions admissions process begins with the submission of our one page initial referral form, usually filled out by the hospital social worker or case manager. The admissions team then reviews the information, and schedules an online or in person interview if it appears our program meets the needs of the potential resident. Our goal is to make sure our program is a good fit and to ensure we are able to provide a therapeutic and safe experience for everyone that we serve. Please contact us for more information.
