Admissions

Our programs are designed primarily to serve people with chronic mental health challenges who are hospitalized but no longer need the level of care available in a hospital. If the situation is appropriate we will take referrals from the community.

The Collaborative Solutions admissions process begins by submitting our one-page initial referral form, usually filled out by the hospital social worker or case manager. The admissions team will review the information. If it appears our program is an appropriate placement for the potential resident, the team will schedule an online or in-person interview. Our goal is to make sure our program is suitable for the applicant, to ensure we are able to provide a therapeutic experience in the program, and to create a safe environment for everyone that we serve.
